• Whether your vehicle or equipment currently has active insurance (yes, no, or not sure).
• Any additional information you include in the notes field.

We do not ask you to submit Social Security numbers, driver’s license images, credit card numbers, or bank information through this website, and we ask that you do not include them in your request.
